About this Divi

When a patient no longer needs care in the hospital, he or she is discharged. Being discharged does not always mean that the patient has fully recovered. Therefore, it is important to prepare the patient for the period after discharge. It should be clear to patients what living rules they need to follow, what they need to arrange for themselves in terms of aids or home care, and where to go with questions. This Divi contains an explanation of this period.

Some patients cannot recover at home and must go (temporarily) to a nursing home. Other patients can go home, but still need support from home care, for example. This Divi also provides information about these different scenarios. The patient views the part that applies to him or her and thus knows what to prepare for.
