When a patient no longer needs care in the hospital, he or she is discharged. Being discharged does not always mean that the patient has fully recovered. Therefore, it is important to prepare the patient for the period after discharge. It should be clear to patients what living rules they need to follow, what they need to arrange for themselves in terms of aids or home care, and where to go with questions.
